Hi!

I am Naraffel Peligrino, I would like to request my Form 2316. I need it for my new job. I was an employee of Jollibee Foods Corporation at Alturas Mall, Tagbilaran City, Bohol. Now, I'm located in Lahug, Cebu City, Cebu that's why I can't get my form 2316 from my previous employer. That would be all, I'm hoping for an immediate response thank you.

Dear Mr. Peligrino:

I refer to my letter of Feb 21, 2023 02:00:22 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR Form 2316.

In reply, please be informed of the following:

1. The requesting party shall attach to his eFOI request Scanned copies of least two (2) valid government I.D.s containing the photograph and signature of the requesting party;

2. Kindly include your Middle Name and Birthday for us to be able to endorsed to the concerned BIR Office/Revenue District Office (RDO) where your employer for 2021-2022 is registered; and

3. Kindly email the needed information at sherry.ann.arcita@bir.gov.ph and indicate in the subject email ticket number #BIR-372701627371, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before May 16, 2023 11:59:05 PM, your request will be closed.

We hope to hear from you at the soonest possible time.
