I need my ITR for the following years:
2017
2018
2019
2020
2021

I have been working in Provincial Government Bulacan-Bulacan Medical Center as a Pharmacist I since October 15, 2017.

Dear Ms. Janelle Blas:

Greetings!

Thank you for your request dated Jul 13, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You request for your INCOME TAX RETURN.

In reply, we have called your contact information today (July 14, 2022) to seek for additional details and clarifications specifically on the following details:

1. Did you have a registered business between the periods 2017-2021? Or A Self-Employed Individual under Contract of Service? Or were you an individual earning on purely compensation income from the period of 2017-2021?

2.  If employed, kindly provide the name of your employer and registered address of your employer for the period 2017 and 2021.

3. Withstanding your answer to the first and second query, did you file or your employer in your behalf an ITR for the cited periods?

You are also advised to coordinate with your employer (2017-2021) as they are the ones who prepare your BIR Form 2316 Certificate of Income and Tax Withheld and issue the same to their employees.

Otherwise, you may try to request a copy of your BIR Form 2316 with the Revenue District Office (RDO) where your employer is registered and submits reports only if your employer submitted the same to them.
