Hi! I would like to request a reissueance of my TIN ID.

My middle name became my surname on my ID.

My Surname is Galanza, and my middle name is Maragrag, but on my TIN ID it is printed as MARAGRAG SHANAIL GALANZA.

It has been mixed up during printing and I only noticed it recently.
