Hi! I requested for my ITR last Friday, and I got a call from you regarding it. It was solved but it's different now since I just saw an email regarding my previous employer that they sent me an application for registration for ITR last 2018. So they really applied for it under my name. I would like to request for the same document if it's possible.  Attached is the said BIR Form No. 1902. If it's possible, I also would like to request for my TIN. Thank you again for your help.

Dear Sandra Mae Clotario:

I refer to my letter of Aug 08, 2022 08:11:58 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return (ITR) and/or BIR Form 2316.

Per our phone conversation this morning August 10, 2022, you are apprised of the following:

1. On your Request for an ITR Copy for the Year 2021- in lieu of your requested ITR for 2021, you may execute the following:
a. Duly Notarized Affidavit of Low Income/No Income (1 original) -  Taxpayer/Notary Public; 
b. Certification of Low Income/No Income signed by the Barangay Chairman of the place where the applicant resides; (1 original) -  Barangay;
c. Proof of Payment of P100.00 Certification Fee and
d. P30.00 loose Documentary Stamp Tax. (1 photocopy) - Collection Section of the Revenue District Office (RDO) having jurisdiction over the place of residence of the taxpayer.

Additional Documents, if applicable to the following cases: If transacting through a Representative: Special Power of Attorney (SPA) executed by the
taxpayer-applicant; (1 original); and Any government-issued ID of the taxpayer and authorized representative; (1 photocopy).

Note: IDs shall be presented and should be readable, untampered and contains consistent information with the documents presented upon application.

2. For your ITR for the taxable Year 2018-Kindly provide us the Name of your Employer (2018), Business Address of your Employer and Employer Taxpayer Identification Number (TIN) and email at sherry.ann.arcita@bir.gov.ph (as this contain sensitive information). Indicate in the subject email tracking number #BIR-388926065396, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Nov 02, 2022 12:36:51 AM, your request will be closed.
