Hi! I would like to request a copy of my BIR Form No. 2316 with my previous employer Analog Devices General Trias Inc on the past 6 years (2017-2022).
My Tax ID Number : 331-357-595.
I'm currently base here in Singapore and no more connection with my past employer. I need those documents as a Job requirement.

You asked for BIR Form No. 2316.
Response to Request
Your FOI request is approved. I enclose a copy of BIR Form 2316 for the years 2018, 2020 & 2021 of the information you requested. Your former employer ANALOG DEVICES GEN TRIAS, INC. did not submit 2316 for TY 2017 and for TY 2019 the DVD-R they submit that contains the 2316 copies of their employee is unreadable, thus LTDPQAD request for replacement but up to now no response from them.

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Ms. Janet Go of LTDPQAD at janet.go@bir.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
