Greetings!

Thank you for your request dated Oct 03, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your request

You asked for BIR FORMS - 2316, 1905 and TIN VERIFICATION SLIP.

Response to your request

In reply, please be informed of the following:

a) TIN Verification Request - you can use the Revie’s TIN Inquiry Module which can be accessed at the BIR website at https://www.bir.gov.ph.

b) BIR Form No. 1905 Request - the said form is available for download at the BIR Website under this link https://www.bir.gov.ph/index.php/bir-forms.html.

c) BIR Form No. 2316 (CY 2018 to 2019) - you may directly request the form/certificate to your employer. Your EMPLOYER is required to issue a copy of your BIR Form 2316 for Calendar Year/s 2018 to 2019 pursuant to Section 2.83.1 of Revenue Regulations No. 02-98, as amended which provides that “The EMPLOYER shall furnish each employee with the original copies of Form No. 2316 xxx.” As proof that your taxes were withheld. This will serve as your Income Tax Return (ITR).
