I would like to request information from Cooperative Development Authority pertaining to:
1. Active Primary Cooperatives classified as Community-based, Savings & Credit and Multipurpose cooperatives in Mindanao based on their total assets and net surplus for the year 2021
2. Please include the head office address of each cooperative particularly the region.
