My name is John Carlo Alavazo, I would like to request for the directory of the active list of cooperatives in the Philippines, separated by regions from January 1 2018 to January 1 2020. May I ask for the following details.

1. Name of Cooperative 
2. Type / Purpose/ Category of Cooperative
3. Address of the Cooperative
4. Contact person with their number
5. Email Address, web sites and/or facebook pages.
