To whom it may concern,

 I was fully vaccinated in Clark. Just want to follow up please update my vaccination record. I have been register several times in vaxcert.ph but it says no record found. Please make an action of this. Since I need the certificate going  to Bohol. Please kindly update my vaccination record so that I can get my certificate then. I hope to hear from you soon. Thank you!

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. However, you may wish to contact the Department of Health Vaccination Team, Clark Polytechnic at 09615411881, or resubmit your request addressed to the correct agency on the eFOI portal at https://doh.gov.ph .

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Gen. Manuel Gaerlan, CDC President. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
