To whom it may concern,

I would like to change my Date of birth on my vaccine card. I've written the wrong DOB when I was signing up for my 1st dose. Please change it from June 18 ,1995 to March 18,1995 I might run into some  problems in the future with any of my government documents. and please include the lot number on my 2nd dose i think they forgot to input that as well
