Good day! This Danica Quiñano, I am writing this letter to ask for an assistance regarding the issuance of my vaccination cert. As I went thru the website of vaxcert.ph it says that no vaccination record was found. 
My vaccination held at CDC Clark Pampang
Primary shots of Pfizer and one booster of Moderna. 

Hoping for your immediate response as I urgently needed it for my flight on Monday.
