Hi I would also like to make the same request regarding:

1.) May I request for a site visitation to Clark Airbase Hospital, regarding to my Thesis Project.

2.) If approved, where can I send the letter for said visit? Is there an email address or online?

3.) What are the necessary documents or requirements for the visitation to proceed?

4.) Can I prepare questionnaire/surveys to them upon my site visitation?
