Good afternoon ..My first dose vaccination was last  June 3, 2021 and got my second dose last July 6, 2021. I  have my vaccination certificate from Bureau of Quarantine (yellow card) and I did my booster shot of astrazeneca last January 13, 2022  in RHU Nabunturan , Davao De Oro with Lot no ACA7295. Can i have a booster certificate?  Thank you so much for your prompt response.
