I would like to raise a concern regarding  vaccination record of my boyfriend's mother. She lost her vaccination card last December. She is fully vaccinated but when I checked her record through https://vaxcert.doh.gov.ph/, it says that they have no record of her first dose vaccination only the second dose were found. 

She was vaccinated in via Antipolo Vacc Operations Center's at Solid Cement Corporation Plant. Her first dose was on August 10, 2021  and the second dose was at September 16, 2021 . As I have said above she lost her vaccination card and wasn't able to take photo of it.
