I would like to raise a concern regarding my vaccination record. I lost my vaccination card this May 2022. I'm fully vaccinated but I don't have other copy of my vaccination card. I was vaccinated in Lapu-Lapu City Island Centre. My first dose was on December 03, 2021 and the second dose was at December 28, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it.
