Dear Mr. Ngo Lee, Thank you for your request dated May 07, 2018 09:02 AM under Executive Order No. 2 (s. 2016) on Freedom of Information (FOI) in the Executive Branch. Your request “I would like to ask for a copy of the Official Receipt of my Passport for my Official Receipt was torn and lost prior to claiming my Passport at DFA Megamall.” Response to your request While our aim is to provide information whenever possible, please be advised that the official receipt being requested is an information of personal, not public, concern. Our FOI policy is guided by Section 1.1 of the DFA FOI Manual, which commits the Department to the full public disclosure of its transactions “involving public interest”. By addressing your personal request through FOI, the DFA is required to upload a copy of your passport’s official receipt on the government’s FOI portal (www.foi.gov.ph), for everyone to see. As an alternative, we advise you to send the DFA a signed letter containing the pertinent details of your request (including the date of your appointment at DFA NCR East, and a copy of your passport data page) to oca@dfa.gov.ph, cc: opd.div2@dfa.gov.ph.
