I am Carmelo Lansangan Loretizo, 42 years old, currently residing in B4 L1 Summerfield, F Manalo, San Isidro Balanti, Cainta, Rizal. I just want to let your office know that I was granted a five year C-1/D US visa. Here are the details of the passport issued;

DOB/ POB : 07 December 1977/ Marikina MM
Passport No. : GG360242

When I renew my passport in 2004, I was not allowed to returned my old passport with a valid C-1D attached to it, in exchange for a new one. If I could get a photocopy of my C-1/D attached to it, I will use it for future reference. Thank you very kindly.
