Hi !
I would like to know details of shipment of my passport to Zamboanga which I applied for with the Department of Foreign Affairs CAGAYAN DE ORO to wit:

APPOINTMENT REF#	: 00152018022400154
SITE NAME	: Cagayan De Oro
DATE	: Saturday, Feb 24, 2018
TIME	: 14:00 - 15:00

DFA-CDO despite repeated requests/demands  REFUSED to give me copy of the TURNOVER MANIFESTO to validate their claim that my passport has been sent out via LBC-Centrio Ayala last March 9, 2018 per tracking no. 179206340208.
Until NOW, NO PASSPORT WAS DELIVERED TO ME.

I badly need your DIVINE intervention. Thank you!

Thank you for your request dated Nov 02, 2019 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your request

You asked for TURN OVER MANIFESTO OF PASSPORT DATED MARCH 9, 2018.

Response to your request

While our aim is to provide information whenever possible, in this instance the information you are requesting even if it is your personal credentials falls under the inventory of exceptions to the right to access information and is confidential in nature. 

Section 4(a) of the Memorandum from the Executive Secretary on Executive Order No. 02 s. 2016 states that the following is exempted from FOI: 

“Information of a personal nature where disclosures would constitute or clearly unwarranted invasion of personal privacy, personal information or records, including sensitive personal information, birth records, school records, or medical or health records…” 

The need to keep personal information inviolable is recognized in international law as well as in the Inventory of Exceptions to Executive Order No. 02. Therefore the DFA is constrained by law from looking into the subject pursuant to the above-stated provisions. 

This Office does not have access to the information you have requested. However, you may wish to contact the Office of Consular Affairs at 8556-0000 or 8651-9400 or by emailing them at oca.concerns@dfa.gov.ph. 

Your right to request a review
