Hello!  Good afternoon! I would like to connect to dfa personal regarding to my  lost passport or can give me any idea about the process and request upon the rules and regulations by processing emergency  lost passport .

Response to your request

Please be advised that your request cannot be categorized as an FOI request. 

However, If you want to apply for the replacement of your lost passport, you need to schedule an appointment at http://passport.gov.ph. 
 
 Please choose "LOST" as the type of passport application.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to opcd.div2@dfa.gov.ph.
