Greetings!
I am writing to request an appointment for passport replacement at any DFA branches. I am currently residing in Cebu, however there are no available slots yet. I will travel to Luzon if there will be an available nearest schedule possible.

I am a nurse and I am scheduled for deployment to Isle Of Wight, United Kingdom this coming October 28, 2021. I am supposed to be scheduled for a Visa appearance this October 7, 2021, unfortunately, my passport has been damaged due to spilled hairspray.

I am hoping for your kind consideration so I can proceed with my application as an OFW.
