Dear Ma'am or Sir,

Greetings! 

I humbly ask for a refund of my passport appointment fee amounting to Php 1,200.00. I was not able to apply for a passport due to some discrepancies in my PSA. Below is my appointment details.

SITE NAME	: 08. SM Seaside Cebu - Temporary Off-site


I was advised to apply for a correction of PSA before I can proceed with my application.

I hope you will consider my request because I am also struggling with my finance. Thank you so much!

You asked for refund for your Passport Appointment Fee.

Response to your request

Please be advised that your request cannot be categorized as an FOI request. 

However, If you have already made your personal appearance for your previous appointment, you are given 30 calendar days to produce any lacking requirements. If your documents are still incomplete after the given deadline, your appointment slot and payment will be forfeited. It is the responsibility of the applicant to make sure that all his documents are correct and complete before setting an online appointment to avoid any inconvenience.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to opcd.div2@dfa.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
