Hi po. My name is John Bryn Q. Gabisay. Mag rerequest po sana ako ng certificate para sa aking booster dose pero nakalimutan ko po yung exact date kung kailan ako nagpa booster at nawawala din po yung aking card. Sana po ay Matulongan nyo po akong ma retrieve itong information na ito. Yung booster ko po ay between June and July of 2022 sa veterans health center QC po.

Hello po, is there any chance na we can get the information sooner than August 8 po? We just really need to get the info cause I'm supposed to be on boarding as a seafarer po. Yung booster certificate lang po talaga young kulang. Thank you po.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
Rest-assured that we've already coordinated with Agent 213 from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Please expect a call or an e-mail from them, if none, kindly call them back through the number above.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
