On behalf of the TULAY Project conducted by DLSU-IBEHT, we would like to inquire if you have a directory of hospitals in the above regions 1, 2, 3, 8, 9, 10, CAR, and NCR in order for us to send them, particularly their medical director, our request for a survey for our research project.
We have attempted to email the regional offices of DOH directly, but some have not responded in over a month.
We would greatly appreciate any assistance on this. An accurate email address or contact number (mobile/landline) would be a great help already.

I have just received a response to my previous request that also answers this one. I was directed to the NHFR website to find the contact details of hospitals we are surveying. If the response for this request would be the same, no need to push through.
