I would like to raise a concern regarding my vaccination record. I lost my vaccination card last month of December. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. 

I was vaccinated in Caloocan City North Medical Center. My first dose was on November 19, 2021 and the second dose was at December 10, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it.

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
