I am Lou Ann Andaya , We requested a vaccination certificate for Mr. John Rey Labrador Ref# 4388735 and Francis Vince Cruz Jr. Ref#4388839 located at City of Santa Rosa Laguna. This is for the application of TESDA purposes only. I would appreciate if you help me out on how to follow up with this as soon as possible. Thank you very much.
