hi!, i would like to request for a copy of my vaccination card. i lost it around 3rd quarter of last year 2023 and i got vaccinated twice already. I don't have a photo of my vaccine card and I also forgot the date of my vaccination but I can provide my basic personal imformation.

Response to Request

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record before the issuance of a new vaccination card.

For further assistance, you may call the Vax Cert Customer Service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.


Your right to request a review
