Im dr. Chrisanthydel tiu, a doctor to the barrio under the DOH, currently taking masters degree on health systems development at development academy of the philippines. My thesis is entitled "rationalization of merged health facilites in the philippines". In line with this, I would like to request for copies of lgu score cards for health from year 2014 to present of the following municipalities: 
1. San mateo,Isabela
2. Sta. Maria,isabela
3. Liloy,zamboanga del norte
4. Jones,Isabela
5. Pangutaran,Sulu
6. Puerto Galera,Mindoro
7. Calayan,Cagayan
8. Sogod,libagon,tomas oppus,bontoc south leyte
