I hope this letter finds you well. My name is Lawrenz G. Caintic, and I am currently in the process of applying for a working scholar at our school. I am writing to inform you that I have unfortunately misplaced my original vaccination card and am in need of a replacement as soon as possible.

I understand the importance of accurate health records and am committed to providing necessary documentation promptly. Please assist me in issuing a new vaccination card to complete my application.

Response to Request

Your FOI request is approved. 
About the copy of vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
