You asked for vaccine card.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. 

We regret to inform you that we do not have custody of the vaccination information and ask that you directly coordinate with the health center or vaccination facility where your mother received her vaccination. We also cannot disclose medical and vaccination records in the eFOI Portal as this constitutes sensitive personal information covered under the Data Privacy Act.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to DOH Central Appeals and Review Committee, at foi@doh.gov.ph. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.


If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
