Hi, my name is Mary Ann Ramos Castro, and I would like to request for a new vaccination card for myself and my 2 children. I lost all our cards recently, and have no back up files. I am fully vaccinated, my first two doses as well as my first booster were dine at the House of Representatives on 06/21/21, 06/18/21 and 12/17/21. My 2nd booster was done on 09/17/22 at the SM Fairview. My son Marcus Theodore Ramos Castro, age 10, and Axel Liam Ramos Castro, age 7, were vaccinated at the Waltermart,  Muñoz Market, EDSA last 09/03/22 and both received their 2nd dose at SM Farview 09/17/22. Pls help.

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
