I would luke to request a copy of my vaccine card. My vaccine was Sinovac and my booster shot was moderna.

About the lost/copy vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
