Goodevening i would like to request information regarding 

1. Specific places that has the highest and latest population of amputees (no. Of amputees in every place/provinces)

2. List of rehabilitation center/facility for amputees (no. Of patient per rehab) 

3. Updated list of places/provinces has highest percentage of HIV in region 4A (no. Of HIV patient in every provinces in region 4A) 

4. No. Of HIV patient per treatment center.

While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested on HIV patients and amputees. We can only provide the HIV surveillance reports posted in the DOH website at https://www.doh.gov.ph/statistics
