Hi there,

May this message bring you good tidings. Hope you may shed the light on OCA and HEA. 
May I ask for status of OCA from April to 2022 from City Health Office-Iligan. We already received our February and March last December 2022 but unfortunately we have news after that because we lose contact from the City Health Office Iligan for dehire.

In addition to, our HEA also we do not have any disclosure about it even the office had a file on our contact number to inform us directly.
