Hi! The baby book of my child (Jose Sebastian Manducdoc) containing his immunization record since birth is missing. He is vaccinated in Pamplona tres Health Center located in Las Piñas City. And now, the school that he is about to enter is asking for it. Could I request for a duplicate copy of her immunization record? Thank you

You asked for Immunization Record of my Son.

Response to your request

While our aim is to provide information whenever possible, we regret to inform you that DOH does not have custody of immunization records. We ask that you directly coordinate with the health center where your son  received his vaccination or you may coordinate with the  Pamplona Tres Health Center located in Las Piñas City or the Las Pinas City LGU for this request
