Good day Ma'am/Sir,

I would like to request to correct my lot number of my first dose in Vaxcert. Currently encoded is 1 instead of L.
Ok naman po siya kapag 1st dose and 2nd dose yong ipiprint na vaxcert. Pero once na kasama napo yong booster don po nag iiba yong lot number ng first dose.Nagpunta nadin po ako ng LGU San pedro para ayusin pero wala naman po silang nagawa may antay lang daw po ng ilang araw pero buwan ang lumipas wala naman pong nabago. Need kolang po talaga siya maayos kaya sana po matulungan niyo ako.Thank you po.

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
Rest-assured that we've already coordinated with Agent 212 from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Please expect a call or an e-mail from them, if none, kindly call them back through the number above. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
