Hello!
I would like to request correction of my name in my vaccination record and certificate on vaxcertph. 
The website / vaxcert record for my name is set to ROWEL A. PERALTA JR as my full name but the correct name is ROWEL A PERALTA. I really need to change my full name asap because I have a scheduled flight.

Response to Request

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Don't worry, I already contacted Agent 182 from the Vaxcert Team and requested for assistance to generate your vaccination certificate. Please expect her to call you on your phone number or you can call them at (02) 8651-7800 local 2432.
