Visit the Health Facilities and Services Regulatory Bureau at https://hfsrb.doh.gov.ph/licensing-accreditation-of-health-facilities-and-services/ for the licensing and accreditation requirements of health facilities and its Permit to Construct of health facility at https://hfsrb.doh.gov.ph/581-2/

Please note that we are only able to provide some of the information you have requested for as we would like to inform you that there is no such health facility as Deathcare Facility. We ask that you review your study with your thesis adviser based on the health facility type indicated on the HFSRB website, then send a new eFOI request.
