I am Ashley Panganiban, a 4th AB Psychology student from Lyceum of the Philippines University Batangas, currently working on a thesis.
Along with this, I would like to request a list along with the age, sex, marital status, address and contact information of PWD with Visual Impairment in Batangas Province and/or those with eye conditions. The records you will give me will only be used for this research alone and the privacy and confidentiality of their identity will remain protected throughout the process.
