Good Afternoon! I would like to file a request for another vaccine card. It was included in the wallet that was stolen from me. I do not have any pictures of it but I got my first vaccine last October 08, 2021, and the second one on December 10, 2021, while still working at Concentrix. I also tried searching for my vaccine record in VaxCertPH, but sadly it was still not recorded online. 

I wish my request will be granted as soon as possible because it was one of the requirements needed by the company that I've applied for. Thank you!

with the LGU or with the company where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
