I would like to ask request on DOH to update my vaccine record since my vaccine certificate does not yet appear. I will be using the details to travel abroad. 

Here are my details:
Origin: Quezon City

1st dose: Sinovac 04/22/21 Quezon City
2nd dose: Sinovac 05/14/21 Quezon City

You asked for Vaccine Certification.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
