Good day! 

I have filed a request on your website last December 21, 2023 for a vaccination certificate and upon checking today, there has still been no progress. The status of my request is still “open”. In line with that, I would like to request for it to be followed up and expedited if possible as I would need to travel abroad this coming February and would need said vaccination certificate.

You asked for Vaccination Certificate No Update.

Response to Request

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet to the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request assistance to generate your vaccination certificate. 
In the meantime, we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer Service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
