You asked for Booster Certificate.

Response to Request

Your FOI request is approved. 

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

 kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your Booster vaccination certificate. 

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
