I would like to request to change of my last name in my vaccination record and certificate on vaxcertph. The website / vaxcert record for my name is set to STEPHANIE S. PLAZA as my last name  but the name in my other documents and ID's is still STEPHANIE C. SAEL which is still my maiden name. I really need to change my last name asap because I have no time to update my last name in my ID's due to my busy schedule. Below are all the details request to change. Last name: Sael

You asked for Change my Last name in my Vaxcert and Vaccination Card.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
