I would like to raise a concern regarding my vaccination record. I lost my vaccination card this month of February. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my 1st and second vaccination. 

I was vaccinated in Saint Peter of Verona Acade Via Resbakuna, Hermosa, Bataan My first dose was on November 02, 2021 and the second dose was at December 03, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
