You asked for Vaxcert booster shot.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have  the information you have requested. 

If you are not vaccinated in the Philippines for any of your doses including booster shot, unfortunately, your vaccination records may not be accessible by the central database which holds the data of recipients of the COVID-19 vaccine from the Philippines. Kindly request for proof of vaccination (i.e. vaccination card/vaccination certificate) from the country where you received your primary dose series and booster shot, provided it has all the necessary information (personal details and vaccination details).

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Undersecretary Lilibeth C.
