Hi ako po si junrey chavez  
Nag rerequest po ako ng vaccines record ko dhil  hindi kona matandaan ang mga datails ko sa vaccin card ko 
Hindi ko matandaan ang date ng  first dose and second dose ng vaccines ko 
Nahulog ko po kasi ito sa may  baseco port area manila po kong saan ako.nag pa vaccine pero niregister nila ang vaccine ko sa my bambang  brgy 264 manila  kylangan ko po sa pag apply ng yellow card thank.u

Kindly coordinate directly with the National Patient Navigation and Referral Center (NPNRC) at Telephone number (02)8651-7800 local 2431-2345 or hotline number 1555 local 2 for immediate assistance.

Please prepare your reference number generated thru Vaxcert.doh.gov.ph for verification purposes and ease of transaction.

Need ko lang po sna malaman anung  year moth and date ng first dose at second dose ko mam para mkakuha ako ng vaxcert thank u po
