I'm martin john sta maria from maria aurora aurora province Philippines I'm vaccinated at ermita manila on Nov 4 2021 but I lost my vaccine card then when I check to vax certificate portal it no record found ..so I make a request to know where can I get a vaccination certificate to use for my job requirements thank you.

You asked for Covid vaccine certificate.

Response to Request

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card.

For further assistance, you may call the Vax Cert Customer Service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.


Your right to request a review
