Hi! I would like to request to correct the surname in my Covid-19 Vaccination card. The name listed is
JHOANA BAYOT NONO and recently my Birthcertificate was updated to JHOANA BAYOT. The (NONO) in the surname is no longer exist. 
I will be needing the correct card for my vaxcert asap for working abroad purposes.

You asked for COVID-19 VACCINE CARD NAME.

Response to Request

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
Rest-assured that we've already coordinated with Agent 201 from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Please expect a call or an e-mail from them, if none, kindly call them back through the number above. 

In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
