November 22, 2023 Dear Kevin Fernandez, Greetings! Thank you for your request dated Nov 21, 2023, 05:58:01 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for VAX CERTIFICATION. Response to your request Since you were not vaccinated in the Philippines, Unfortunately, the vaccination records may not be accessible by the central database which holds the data of recipients of the COVID-19 vaccine from the Philippines. Kindly request for proof of vaccination (i.e. vaccination card/vaccination certificate) from the country where you received your primary dose series, provided it has all the necessary information (personal details and vaccination details) For further assistance you may review the FAQ section of the eFOI site: https://www.foi.gov.ph/help. Your right to request a review. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the Central Appeals and Review Committee, at foi@doh.gov.ph. We will complete the review and tell you about the result within 30 calendar days from the date when we receive your review request.
