You asked for VAX CERTIFICATION.

Response to your request

Since you were not vaccinated in the Philippines, Unfortunately, the vaccination records may not be accessible by the central database which holds the data of recipients of the COVID-19 vaccine from the Philippines. Kindly request for proof of vaccination (i.e. vaccination card/vaccination certificate) from the country where you received your primary dose series, provided it has all the necessary information (personal details and vaccination details)

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the Central Appeals and Review Committee, at foi@doh.gov.ph.
