While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. We only have the DOH National Health Facility Registry (NHFR) which is the mastertlist of all health facilities at its website: https://nhfr.doh.gov.ph/rfacilities2list.php
